What is your process for hacking?

I ask mostly because I'm getting back into Super Metroid Hacking after... a year? More or less?  ...and I don't know where to start. I was going to start with the map, but then I have to think of where all the items will go, and how the progression goes....

Basically, where do I start with a new hack?
